To Sign Up For Online Web Therapy with Renew Your Life Counseling, L.L.C., the Declaration of Practices and Procedures Form and Notice of HIPAA Privacy Disclosure Form Must Be Signed and Dated, and the Intake Form will also need to be filled out.

You may use the e-sign button to digitally complete and submit the forms or you may download the PDF versions. For the PDF forms, you may complete and sign them and email them to bdarwish@renewlifetherapy.net.You may also manually sign them and scan/email them, or you may fax them to 504-667-6620.
